Our goal is to provide you with the information and tools you need prior to your arrival at The Hip Treatment Center, as well as answer any questions or concerns you may have.
We ask that you please arrive at least 15 minutes early to your appointment to fill out our patient registration forms in the event they have not been filled out in advance. If you’d prefer to print and complete the forms, please call 404-255-1933 to request them.
